About OnPoint Performance
The provider you wish you had the first time.
OnPoint Performance is a solo-practitioner concierge telehealth practice. One provider. No rotating panel. No twelve-minute visits. No reference ranges that miss what actually matters.

Gretchen Delgado
I'm Gretchen Delgado, a board-certified Adult-Gerontology Acute Care Nurse Practitioner. Before founding OnPoint Performance, I spent years in hospital-based acute care watching patients arrive with conditions that had been called "normal" by their primary care providers for years. Fatigue. Brain fog. Low libido. Weight that would not come off no matter what they did. All called normal until they were not.
OnPoint Performance is built on the idea that "your labs are fine" is not the same as "you are well." Reference ranges exist to flag disease. Optimal ranges describe how you should actually feel. Most providers stop at the first one. I work at the second.
My patients are high-performing professionals, athletes, and people in the second half of their lives who refuse to accept gradual decline as normal aging. We work from comprehensive labs, not symptoms alone. Every protocol is built around your bloodwork and adjusted as it evolves. One provider, the whole way through.
Credentials & Licensure
"Most of the people I see have been told their labs are normal for years. They are also tired, soft, foggy, and unrecognizable to themselves. Both things can be true. The job is to fix the second one."Gretchen Delgado, MSN, APRN, AGACNP-BC
How I practice
01
Labs first, always.
Every protocol I prescribe starts with comprehensive bloodwork and a thorough review. No assumptions. No template plans. No "let us try this and see." If your labs do not point to the answer, I keep digging until they do.
02
Optimal, not just normal.
Reference ranges are the lowest bar of health. They exist to flag disease, not to describe wellness. I measure your labs against optimal ranges that actually reflect how a healthy person at your age should feel.
03
One provider, the whole way.
You see me every visit. Not a rotating panel of nurse practitioners. Not a different provider every quarter. The same person who reviewed your last set of labs reviews the next one.
04
Long-term, not transactional.
Hormone optimization is not a one-time prescription. Your body changes. Your labs evolve. Your protocol gets adjusted as you go. The work is not "fix it once." The work is "keep it working."
Work with Gretchen.
Start with the $250 initial assessment. Comprehensive bloodwork, a full clinical review, and a treatment plan built around what your labs actually show.
